If you would like to become a member, please download the appropriate application, fill in all fields, and upload it using the form below. You may also return it to any of our offices during our open business hours.
Standard
Membership
Open to individuals.
Provides full membership with voting rights. Regular members may not be a member of any other church.
Associate
Membership
Open to individuals.
Associate members may not vote or hold elective office. Associate members may hold membership in other churches and religious organizations.
Institutional Affiliate Member
Open to organizations and Institutions.
The designated member may serve advisory and consultative positions on boards and committees but may not vote or hold elective office.
